1. What is HIPAA?
HIPAA is a federal law that sets national standards for protecting your health information. It gives you new rights regarding your medical records and controls how your information may be used or shared by healthcare providers.
2. Your Rights Under HIPAA
You have important rights regarding your health information, including the ability to:
- Access your medical records under most circumstances
- Request amendments or corrections to your record
- Request restrictions on how your health information is communicated
- File a complaint about privacy violations
- Receive a list of disclosures showing who has received your health information
3. How We Protect Your Health Information
We follow strict policies and procedures to ensure your Protected Health Information (PHI) remains private and secure.
PHI includes any information that can identify you, such as:
- Name, address, birth date, or age
- Medical records, diagnoses, lab results, and prescriptions
- Billing and insurance records
- X-rays, photos, and other images
Only authorized personnel with a legitimate need-to-know can access your PHI.
We do not share your information unless required or permitted by law, or with your explicit permission.
4. Confidentiality & Staff Responsibility
All staff are trained to maintain patient confidentiality at all times:
- We discuss patient information only with those directly involved in your care and only in private settings
- Information about minors is shared only with verified legal guardians
- No information is shared with friends or non-authorized individuals, even if requested in person or by phone
- We verify the identity of anyone requesting patient information before sharing any details
Improper access, discussion, or disclosure of PHI is a serious HIPAA violation and may result in disciplinary action or termination.
5. Common HIPAA Violations (What We Never Do)
- Texting or sharing patient information with anyone not directly involved in care
- Discussing patients in public or semi-public areas
- Sharing details with unauthorized staff or outsiders, even with good intentions
- Accessing medical information for curiosity or non-care-related reasons
6. Reporting a Privacy Concern
If you believe your privacy rights have been violated, or if you have questions about our privacy practices, you may contact us through our contact page.
We take all reports seriously and will respond promptly.
7. Policy Updates
Cuba Memorial Hospital may update this HIPAA policy as needed.
Changes will be posted on this page with an updated modification date.
At Cuba Memorial Hospital, patient privacy and confidentiality are at the heart of our care. We do not tolerate HIPAA violations.
